There's a different between game theme and game playability. The inability to teleport cities, the need to find actual space to store millions of troops or billions of food/stone/etc., the need to take months or years to construct buildings, or weeks for scout reports, the fact that food can go bad or become diseased, the fact that barbarian cities should be reduced to ruins instead of being perpetual food farms, etc. etc. to reflect realism, would greatly inhibit playability.
Avatars are purely cosmetic value and have no effect on the game. Therefore I imagine, when putting together a game with a theme faithful to medieval Europe yet still with interactive game mechanics, pretty pictures were very low on Evony's priority list compared to other things (IE; the ability to move cities).
