Halloween is the day before All Saints Day. It's oringinal custom was that kids would dress up as their favorite saint and colect candy from people. However, it has been horribly twisted around and made into another "day of the dead". Notice the term "Hallow". That's another word for holy.

Saint Nicolas is the answer to the first question; I'm pretty sure that either the Netherlands or the UK brought it to the US. And I'm not sure about the last one
